Questionnaire to help an MHort student

A request for help from an M.Hort student,  Crissy Clemson:

Good afternoon, I am emailing in the hope that your club members may be able to help with my dissertation research, by filling in a short online questionnaire. To fill in the form just click on the 'fill out in google forms' link.

For the final part of my MHort, I am writing my dissertation on Plant Labelling in the UK and I would be very grateful if you could take a few minutes to fill in the following questionnaire and return it to me as soon as possible. All of the information provided is anonymous, and will be used to create a picture of the sort of details plant buyers (like yourselves) feel should be included on plant labels.
